Health Educator
Icahn Mount Sinai School of Medicine
Strength Through Diversity
Ground breaking science. Advancing medicine. Healing made personal.

Roles & Responsibilities:

This effort requires a candidate with strong inter-personal communication skills who can develop rapport quickly with patients in a challenging clinical environment.
• HCV Linkage Navigation: The Health Educator will engage with and follow up with all patients who screen positive for HCV in the ED. Health Educators provide patients with their test results and schedule them for their first outpatient clinic appointment. These activities require close coordinating with clinic staff, care managers, administrative staff and other personnel. Follow-up may occur over the phone or in person. Candidates must be able to work effectively in a mul
