Peer Support Specialist - HIV Day Center

This individual will work in collaboration with CAP and Ecumenical Ministries of Oregon’s HIV Day Center staff to support clients experiencing homelessness or housing instability to access needed community resources including housing, healthcare, mental health/substance use treatment, etc. This also includes providing advocacy on behalf of clients with housing and healthcare providers, person-centered goal planning, assistance navigating various systems of care and providing information and referrals to other services as needed. This person will also work with clients to reduce stigma and isolation and to build social connections through participation in the mentoring program and through referral to other psychosocial supports. This is a full-time, 1.0 FTE position, which reports to the Manager of Supportive Programming. The person in this position will be co-located at EMO’s HIV Day Center and at CAP’s Davis St. office.
Cascade AIDS Project CAP’s Service Center Coordinator position is responsible for providing information, referral, and other duties for clients who visit CAP’s walk-in service center. The Service Cent... er Coordinator will provide support to individuals and families who are not fully engaged with housing case management or Service Navigation programs at CAP, but are in need of immediate assistance or referral to other community partners. This position will play a crucial role in the day to day operation of the Service Center, and will often be the first point of contact for new clients. The Service Center Coordinator helps maintain the safety of the space. The Service Center Coordinator will operate as a part of a coordinated team who will perform duties including, but not limited to: answering and returning information and referral phone calls; scheduling and performing intakes; creating goal plans with clients; providing assistance with applications for CAP and other housing programs; communicating client needs and providing referrals to responsible CAP program staff and community partners; and providing client referrals to partner agencies. The Service Center Coordinator is responsible for providing back-up support to the Employment Services program at CAP. This could include assisting with resumés, cover letters, and job applications, and maintaining. This position will also be responsible for tracking client interactions and needs in the agency databases, ServicePoint and CAREWare, with an emphasis on providing accurate and timely documentation. In addition, the Service Center Coordinator will provide task supervision of program volunteers and interns as needed; collection and coordination of client eligibility documentation; and other duties as assigned. This position primarily works with the Manager of Prevention and Integrated Services to support CAP’s PrEP Navigation and HIV/STD testing programs in SW Washington. The person in this position is based in the CAP SW Washington office in Vancouver, WA, but will spend some time at CAP’s Longview office and other sites throughout the Portland Metro area. Some evening and weekend work is required. This is a full-time, non-management, union-represented position.
The position responsibilities include: maintaining a PrEP caseload of 60-75 clients, conducting HIV/STD testing at the CAP SW Washington STD Clinic, outreach testing sites as needed (including Cowlitz, Wahkiakum and Skamania counties) and remotely supporting clients with at-home testing. Testing shifts will include weekly Tuesday and Thursday clinic shifts and one-time event-based outreach testing and supporting remote at-home testing. Data collection and reporting of HIV/STD testing sessions through an Electronic Medical Record. Monitoring and managing HIV testing supplies and HIV testing quality assurance duties. Conducting community-based outreach activities in order to recruit into evidence-based prevention services: HIV/STD testing & treatment, PrEP Navigation, linkage to care services, and safer sex material distribution. Attend Clark County specific meetings including networking events and other associated community meetings, build and maintain collaborative professional relationships with agencies that serve MSM and other priority populations.
HIV Alliance The Nurse Case Manager will administer medical case management services to clients enrolled in the Supportive Housing Program. The Supportive Housing program provides intensive wr... ap-around supports to individuals facing significant barriers to obtaining and maintaining stable housing, often with a history of behavioral health and/or substance use barriers. The ideal candidate will be comfortable working with these populations. The Nurse Case Manager may often travel to deliver medical case management in the home in order to further reduce clients’ barriers to accessing care. The Nurse Case Manager will promote viral suppression and increased overall wellness and stability for Supportive Housing clients.
The Nurse Case Manager will work in close partnership with HIV medical providers in the community. This position will communicate with providers regarding the client’s case management care plan and the medical treatment plan. In addition, HIV medical case managers administer services that many HIV doctors have limited time to provide: treatment management and medication adherence counseling, HIV disease and side effects management, nutritional therapy, oral health interventions and referral, liver health and hepatitis disease management (acute and chronic), HIV risk reduction, STI screening and sexual health education.

The role of the Supportive Housing Navigator includes assisting clients in finding housing placement, enhancing and developing client life skills, creating and reviewing client budgets. The Supportive Housing Navigator will link the client to specific programs as needed such as health insurance, social security benefits, and other medical, and social assistance programs. Ongoing advocacy and support are provided to the client in order to maintain access to medical assistance programs, and minimize any break in access to medical care. Position Summary The Intake Coordinator is the first contact for new clients referred to Ryan White Case Management Services. This position conducts the initial screening with all new clients. In addition, this position conducts the bi-annual phone screening with all clients. The Intake Coordinator also assists clients in obtaining required documentation, as appropriate. This position is responsible for scheduling the first appointment for all new clients with a Care Coordinator and with a Medical Case Manager to complete their assessment process. This position also refers to the Care Coordinators and Medical Case Managers as appropriate when clients need additional assistance after the annual screening is completed.
