Clinic Case Manager

Position Purpose
The Clinic Case Manager provides a wide range of intensive, client-centered services to persons with HIV/AIDS within a clinic setting to ensure access to and retention in healthcare, and to address medical, psycho-social, and/or other issues that present obstacles or barriers to care. The Clinic Case Manager is part of a care team that includes physicians, nurses, nurse practitioners, behavioral health therapists, dental care providers, and other care providers. The Clinic Case Manager is an integral part of Vivent Health’s Medical Home, working as Care Coordinator and Team Leader for enrolled patients.

Position Summary:
Team Damien is currently seeking a Nurse to join our team. The Nurse will be responsible for creating an environment of patient-focused care, defining standards for quality care and leading, and training new and existing staff. This position will report to the Clinical Director and assist in administrative and clinical supervision.

TEST COUNSELOR & PREP SPECIALIST, MIAMI

Latinos Salud is seeking qualified individuals for the position of HIV/STD Test Counselor and PrEP Specialist. Candidates must be bi-lingual (English / Spanish). Strong preference for gay Latino candidates who can personally relate to the primary populations served.
Working primarily from the Miami Beach location, the full-time staff member will:
• Provide HIV and STD counseling and testing to eligible clients, especially gay Latinos;
• Provide education about PrEP, and help navigate interested clients into patient assistance or co-pay programs;
• Provide continued support to those taking PrEP, and to those taking
medications for HIV;
• Provide assistance enrolling clients in other needed support services;
• Provide on-line and venue-based outreach, some evenings and
weekends each month;
• Input and report accurate data in a timely manner;
• Participate in other agency programs and events serving the community.
Starting salary $16.50 to $18.25 per hour, depending upon experience and education. Latinos Salud’s employees receive benefits: health insurance, tuition assistance, a retirement plan, and a generous leave package. Bachelor’s degree preferred, and work experience in a position with direct client contact.
Candidates must complete the Florida 500 / 501 HIV testing certification. Must have own transportation, and be willing to work a flexible schedule.

• Provide one-on-one HIV/STD education and testing to eligible clients.
• Provide assistance enrolling clients in needed support services;
• Provide education about PrEP, help navigate clients into patient

Salary $40,000 – $48,000, depending upon experience and education. Latinos Salud’s employees receive benefits: health insurance, tuition assistance, a retirement plan, and a generous leave package. Bachelor’s degree preferred, and at least two years’ work in a position with direct client contact. Candidates must complete the Florida 500 / 501 HIV testing certification.
