The Brooklyn Hospital PATH Center is seeking a full-time licensed social worker (LMSW or LCSW) to join the Family Program team. The social worker provides psychosocial assessment, medical case management, short and long-term supportive counseling, crisis management and referral services to HIV-positive and high-risk clients within the PATH Center’s Family Program. The social worker works with a multidisciplinary team and manages a caseload of medical case management and therapy patients consisting of adults, adolescents and families. The social worker ensures continuity of care with a focus on positive medical outcomes and completes referrals for outpatient, inpatient and community-based services to address patients’ holistic needs. *This is an 1199 Union position with full benefits. Peer Engagement Specialist
Lantern Community Services
The Peer Engagement Specialist (PES) is responsible for providing assistance to residents in recovery to live as independently as possible within a supportive housing setting. Using lived experience, the PES will help to engage residents in services and activities both internally at Lantern, and in the external community. COVID-19 Response Director (CRD)
Latino Commission on AIDS
COVID19 Response Director: The CRD will oversee the entire COVID-19 response at the Latino Commission on AIDS (Commission) at the local, regional and national levels. The CRD will be responsible for the planning and implementation of several of the Commission’s COVID-19 projects to address the current and emerging needs for prevention, care, and long-term healing due to coronavirus in Hispanic/Latino communities. This manager will work with an internal team and a coalition of partners to implement grant-funded projects in New York City, New York State and nationally.
The CRD will be hands-on on all components of our COVID-19 work, supervise staff and work with internal staff. This position will facilitate the COVID-19 internal taskforce within the Commission. The taskforce works closely with the VP of Programs and other members of the executive management team of the agency to implement the existing projects.

Case Manager
Latino Commission on AIDS
The Oasis Case Manager will concentrate on case management documentation and follow-ups. In addition will provide support in the provision of testing, screening and benefit navigation services.

RESPONSIBILITIES:
- Apply strength -based counseling, motivational interviewing on counseling services along with other strategies recommended by the supervisor
- Perform HIV, HCV, Syphilis and any forthcoming rapid/confirmatory test to be incorporated into our services
- Link clients to HIV care services and supportive services as apply
- Strong participation on OASIS Social Media platforms

- Conduct outreach activities
- Promote agency-wide development initiatives

PREFERRED QUALIFICATIONS:

The OASIS Navigation Coordinator will be in charge of the provision of testing, screening and navigation services. This position will follow the HIV Navigation Services model. Mayor efforts will be allocated on internal and external navigation services.

Operations Manager
The New School
The Center for New York City Affairs is an applied public policy research organization based in the Milano School at the New School. The Center offers analysis and solutions to pressing social and economic challenges.  In early 2017, the Center launched the Institute for Transformational Mentoring (ITM), a dynamic training program focused on the development of credible messengers – formerly incarcerated mentors – working in the social services fields throughout New York City.  These mentors help young people navigate community violence and avoid the criminal justice system.  The Institute was developed in partnership with social service employers and responds to the need for professional development and support for this vital workforce. The Institute also offers programs for young adults seeking to develop their skills as peer mentors.


The Operations Manager is responsible for programmatic and administrative logistics.  This position is a critical member of a small team where staff members work collaboratively and are often expected to pitch in and support each other on a wide range of projects. This position reports to the Co-Directors of ITM and is expected to regularly communicate with both the Executive Director and the Director of Operations for the Center for New York City Affairs.

ITM Education Coordinator

The Data Analyst will establish clinic data systems andworkflow that identify PWH clients determined to be out of care or at risk of falling out of care; works withthe Brookdale IS Epic team to add required variables to the EHR and to develop EHR alerts for automated referral of PWH in the ED and inpatient units; review STAR-Brookdale’s 18-month look-back report on a monthly basis to identify a list of PWH clients who arethought to be OOC (based on adherence to medical/lab appointments, and VL). Work with pharmacy, front desk staff and
Epic team to develop a monthly report of patients RFC. Work with ACE and COTA teams to triangulate developed list with databases external to the clinic including, i.e. RHIOs and CSR to identify clients reengaged elsewhere or deceased. Participate in bi-weekly case conferencing with ACE and COTA Teams to triangulate data from governmental databases (e.g., eHARS) and finalize a list of PWH thought to be
OOC, NIC, or RFC for outreach. Create datasets by combining data from disparate data sources for clinical and data QI activities. Develop a system for collection of all eShare/RSR clientlevel data and other data required by the DOHMH for program evaluation; and ensure the timely submission of all data in eShare, as required.

High school degree or equivalent with technical training and 3 years of data management experience or BA/BS. Experience in data management, reporting, analysis, andquality assurance activities for large scale service projects. Demonstrated experience with spreadsheet and database software; ability to present information inan easily understandable format; and experience working collaboratively with program staff. Experience extracting and managing medical information from electronic health record (EHR) strongly preferred.
