Under the supervision of the Operations Director, oversees and manages the daily operations of the St. Paul’s PACE program including operations of the Day Center, Clinic, Social Work, Rehabilitation, Recreation, Home Care, Transportation, Dietary and Medical Records. This includes ensuring that appropriate personnel are trained and perform their functions within the organization. Duties include assuring that operational services standards and budgetary criteria are met. Additionally ensures operations are in compliance with all governing regulations. Collaborates with other St. Paul’s PACE sites to ensure standard practices across the organization and lead Quality Improvement initiatives. To ensure continuity of operations, assumes the Operations Director’s responsibilities in her/his absence.

Essential Functions:
1. Conducts an in-person initial comprehensive physical therapy assessment on all participants upon enrollment. Coordinates with the Interdisciplinary Team to develop a comprehensive plan of care for each participant.
2. Conducts an in-person physical therapy re-assessment minimally every six (6) months or as needed.
3. Functions as a member of the Interdisciplinary Team. Maintains regular attendance at, and participates in, Interdisciplinary Team meetings; communicates participant changes, collaborates on plan of care decisions and coordination for twenty-four (24) hour care delivery.
4. Provides treatment to participants and implements physical therapy procedures consistent in achieving identified goals. Grades activities to facilitate goals. Confers with Interdisciplinary Team on participants care issues.
5. Evaluates participant’s needs. Follows-up with participants during treatment, explaining precautions to take. Discusses condition and treatment plans and recommends solutions to participants care problems with Interdisciplinary Team.
6. Assesses the need for adaptive Durable Medical Equipment (DME), specialized shoes and prosthesis followed by recommendations to Interdisciplinary Team. Receives authorization prior to purchases. Assesses all newly issued orthotic shoes, braces and prosthetics for appropriateness and proper fit.

Redwood Coast PACE/Humboldt Senior Resource Center
The Redwood Coast Program for All-Inclusive Care for the Elderly is looking for a full time primary care physician to join our team. The PACE program is a national model of care, & we are the first rural program in California, opening in September of 2014. We have two sites & our program continues to grow. The position is Monday-Friday 8:30-5, with on call responsibilities to back-up nurses and nurse practitioners on first call, with occasional first call responsibilities. The model is high intensity, low productivity care. We average 4-8 visits per day, in clinic, home visits, RCFE visits, SNF visits & occasionally in hospital visits. We have an interdisciplinary team approach, with social work, nursing, dietitian, recreational therapist, physical & occupational therapists, transportation & home care. Applicants for this position should have comfort with high acuity medical issues, preferably with some inpatient training & experience. They should also be comfortable with end of life care, palliative care, & mental health care. Team work & communication is essential. We interact often with specialist & hospitalist colleagues, adult protective services, & participant families. One must be self-directed & comfortable with a variable schedule, as we must respond to the acute needs of our participants as well as to their chronic primary care needs. One must also have strong computer skills. This position may require splitting time between our Eureka & Fortuna site. The work at PACE is deeply satisfying & in many ways what most of us pictured when we decided to go into medicine. We look forward to speaking with interested candidates.
