This position is a hybrid of teleworking and client visits in the community setting including client homes, facility settings, school/day programs, and hospitals.

Facilitate the development of individualized client Care Management Plans.
Locate, coordinate, and navigate medical services and other community-based services.
Research and recommend cost-effective alternatives to care.
Engage best practices to guide client service delivery to include assessments, documentation, planning, implementation, education, advocacy, and evaluation.
Facilitate communication providers, insurances, the multidisciplinary team and supports required by the client.
Coordinate quarterly on-site visits with clients and maintain monthly contact with clients.
