include but are not limited to providing enhanced patient navigation services to identify and link eligible participants to
services; Patient Navigator will offer participants referrals and linkages for all necessary services, MAT, and other ancillary service needs.
Participate in outreach efforts at community sites and in-reach at TTC service sites; provide follow-up as needed to
engage potential participants or those who have fallen out of care. BA/BS preferred with 3-5 years customer service
experience in related fields; equivalent related experience may be substituted for degree. Candidate will have at
minimum two years relevant experience within Healthcare services/promotion, community-based organization,
outreach, Intake coordination, and/or social services setting. Candidate should have a working knowledge of
substance use disorder and mental health treatment services, and Integrative care. Experience working effectively with culturally, socio-economically, and ethnically diverse populations.
Bilingual English/Spanish preferred. Ability and willingness to work a flexible schedule. Culturally competent with an
interest in working with target population.
