Behavioral Health Provider (LCSW/LPCC)

The Behavioral Health Provider, as a key part of La Familia Medical Center’s (LFMC) primary care treatment team, identifies, triages and manages patients with behavioral health problems within the primary care setting. In addition, the Counselor/Social Worker will provide skill training through psycho-education and patient education strategies and will develop specific plans for a patient’s behavioral health assessment, treatment and referral.
