Finally, they will manage vendor contracts and work with Esperanza's external affairs team to maintain positive public relations for the program.

Esperanza Health Centers The Medical Director of PACE is responsible for providing clinical leadership for the care and treatment of participants served by Esperanza's Program of All-Inclusive Care for th... e Elderly. This position provides clinical direction and guidance to the PACE program and staff to assure the maintenance of quality care for participants and families. They are ultimately responsible for medical outcomes and serve as a liaison between the program and Lommunity physicians, as well as with others engaged in health care services. This position and/or their designees provide direct medical care as needed to participants in the home, the PACE center, and in other multiple healthcare settings. This position supervises the PACE program's medical staff and those served by the interdisciplinary Care team (IDT). The Medical Director of PACE reports to the Chief Medical Officer.

Educate participants and families regarding health maintenance and their chronic medical conditions, in cooperation with nursing staff. Serve as a member of the interdisciplinary team and participate in the development of the comprehensive plan of care and ongoing monitoring of the participant’s health status. Maintain an active role in the interdisciplinary team process. Candidates will have:
Must be licensed to practice medicine in the state of Indiana. Experience or added qualifications in geriatrics preferred. Board certified/eligible in internal medicine or family practice. Advanced certification in geriatrics and Board certification granted by the American Boards of Internal Medicine and Family Practice for Added Qualification in Geriatric Medicine preferred. Must provide current state license and DEA registration and obtain staff privileges at the Saint Joseph PACE contracted hospitals. Minimum of one (1) year of documented experience working with a frail or elderly population. Interest in frail older adults and knowledge of their physical, mental, and social needs.
