This position will be responsible for providing behavioral health support at the Abraham and Lillian Turk Recuperative Care Center which provides short-term post-hospitalization discharge care and support in a medical respite setting to individuals, including Veterans who are experiencing high medical, psychiatric, and substance use disorder recovery needs and are experiencing homelessness or are in unstable living situations that are too ill or frail to recover from an illness (physical or behavioral health) or injury in their usual living environment; but are not otherwise ill enough to be in a hospital. Post-discharge treatment in a medical respite setting allows individuals to achieve or maintain medical stability and prevent hospital admission or readmission and achieve a more adaptive level of psychosocial functioning while in the program. The clinical therapist, with support from the program manager provides short term therapy, crisis intervention, and support for clients with a short-term recuperative need in a residential program.

This position will provide professional mental health services, to include case consultation and individual and group psychotherapy; evaluations and assessments, and perform related work as required supporting the mission of Interfaith Community Services.
The Recovery and Wellness Center was created with the vision that all individuals should have an opportunity to pursue wellness and recovery and in turn experience an improved quality of life regardless of their ability to pay. Interfaith will provide a recovery-oriented system of care that is designed to support individuals seeking to recover from substance use disorders and/or co-occurring conditions across their lifespan. The RWC treatment center will include withdrawal management (detox), residential and outpatient treatment services.

Conducts intake assessments; prepares diagnostic evaluations which includes obtaining personal, social, emotional, and substance abuse history; determines the cause, nature, and severity of the client’s current mental status
Determines medical necessity and level of care placement for Substance Use Disorder treatment services
Develops, evaluates, and implements goal-oriented treatment plans
Provides direct treatment services such as individual, family and group therapy to a difficult and complex client population
Intervenes in crisis situations when a client exhibits acute psychiatric symptoms which present an immediate threat to the client or others
Provides rehabilitative and socialization services to restore and maintain the best possible functional level of clients with chronic psychological impairments
Provides consultation to other staff members on a client’s history, mental status, and previous treatment, and on the community resources available for the treatment of a client
Participates in staff review of a case(s); recommends changes in therapeutic techniques, environment, and/or charting to facilitate client recovery

Under the general supervision of the LifeSpring Program Manager, Housing Specialist is responsible for monitoring residents of the TYH and THP Plus program. Monitoring TYH and THP Plus Program residents will take place through assisting in development of a sense of community that focuses on safety, providing immediate and/or timely intervention around situations involving residents that are non-compliant with rules and expectations, providing assistance in conflict resolution, and providing crisis intervention during the evenings and alternating weekends that includes immediate notification of supervisory or on-call staff.  Housing Specialist will conduct apartment inspections at least twice a month and will conduct spot inspections as requested by Supervisor to ensure unit cleanliness and monitor for safety.  Responsible for adhering to landlord-tenant state laws and maintain regular communication and collaboration with property manager to identify potential tenant issues.  Services are to be provided in a trauma informed, client centered, culturally competent manner that is consistent with Lifeline’s philosophy and mission.

Provides individual and/or group supervision to interns and associates within the agency
Provides direct services to designated clientswhich include: intake, assessment, and counseling as well as client advocacy with the goal of supporting self-sufficiency
Recruit, screen, place and supervise interns who will provide short term counseling and case management services at multiple Interfaith service locations.
Working with the agency’s treatment team of behavioral health providers, provide/organize clinical in-service staff trainings to ensure best practice and consistency across programs.
Support treatment team meetings for clinical and/or paraprofessional staff to ensure compliance, best practice and offer clinical supervision for staff working with difficult clients.
Provide direct services to clients including short term counseling, crisis intervention, and linkage with longer term services.
Participate, when appropriate, in case conferences for clients receiving counseling services.
Develop and maintain effective relationships with mental health providers to ensure that clients with ongoing treatment needs are successfully transferred to longer term services.
Monitor compliance with grant and contract requirements. Make recommendations to Supervisor regarding any program changes needed to meet grant benchmarks.
Ensure the confidentiality of client information and health records.
Meet regularly with Supervisor and participate in agency meetings and staff trainings.
Other duties as assigned by Supervisor.
