Father Joe’s Villages Psychiatric mental health nurse practitioners (PMHNPs) provide care to individuals with mental health needs through assessment, diagnosis, and treatment. PMHNPs are nationally board-certified by the American Nurses Credentialing Center (ANCC). PMHNPs at the Village Health Center provide outpatient mental health services to persons experiencing homelessness (PEH).

Father Joe’s Villages This position supports the work of licensed clinicians by obtaining vital signs, administering medications and injections, recording information in electronic medical records, preparing and handling medical instruments and supplies, and collecting and preparing specimens of bodily fluids and tissues for laboratory testing.
Father Joe’s Villages This position contributes to clinic quality assessment and improvement activities and projects, including the reporting of quality outcomes to various internal and external entities. The incumbent holds primary responsibility for provider credentialing, onboarding, and provider scheduling, and provides subject matter expertise on the electronic health record (EHR) systems within the Village Health Center, a Federally Qualified Health Center (FQHC).
Transition of Care RN
MLK Community Healthcare The role of the Transitional Care Nurse is to perform follow up contact with patients within a reasonable time period with the focus on the identification of any continued needs that may exist. The intention of the post discharge call is to ensure that the member has everything needed to recover safely in the home setting, to identify any complications/issues that the member may be experiencing, and to ensure that appropriate follow-up appointments are scheduled and that a plan is in place for member to attend the appointment. Transitional Care Nurse will work closely with Manager of Case Management regarding clinical needs.
