Will provide primary care to geriatric population along the long term continuum with focus on Home visits and Assisted Living Facilities , mainly in Bergen County, NJ .
Experience of at least 1 year in this setting required but will also receive hands-on orientation and training.
Will be providing comprehensive primary care and treat common diseases and medical conditions as well as provide supportive care throughout the end of life stage .
Needs strong communication skills , will work closely with nursing staff and families while building strong bonds with our patients.

Graduate of an accredited Psychiatric Mental Health Nurse Practitioner Education Program
