Position Summary

We have an exciting new opportunity for a Licensed Clinical Social Worker (LCSW) to join a team of highly skilled clinical social workers and provide medical social work and behavioral health support to adult patients at our outpatient oncology clinic. You will be joining a multi-disciplinary, collaborative team who meet weekly to process cases, support each other’s work and empower one another. The team takes turns to go to clinical trainings and has supportive leadership. Prior oncology experience is a plus and we will offer training to the right candidate.

This is a position for the professional social worker that involves the evaluation of psychosocial needs of patients/clients and their families, and provision of diagnostic/preventive and treatment services including individual, family, and group therapy. Also provides complex discharge planning, coordinates aftercare services, and makes referrals for other community services based on the treatment plan. You will work in collaboration with other health/mental health professionals and provides consultation to the treatment team regarding patient/family social and environmental needs. Work is performed under general supervision in accordance with institutional and social work/clinical counselor policies. May supervise other social workers, students, or other staff.

Purpose:

Licensed Clinical Social Worker - Oncology

This full-time clinical position supports the UCHealth Memorial Hospital Cancer Center ambulatory outpatient program in the Colorado Springs Region.

Responsibilities:
- Conducts comprehensive patient assessments and partners with patient to establish and implement patient-tailored goals designed to provide appropriate support, resources, interventions, and referrals, from initial diagnosis through treatment and end of life.
- Implements counseling/therapy for assigned patients (via in-person, virtual and telephone treatment delivery modalities).
- Designs and facilitates patient and/or caregiver support and psychoeducational groups.
- Conducts outreach, and implements appropriate interventions, for patients who have completed the Distress Screen tool, which identifies patients’ self-reported emotional, social, health and practical concerns.
- Provides resources for common concerns like transportation to and from medical appointments, lodging and travel related expenses, financial resources, and information about local, state, and national resources.
- Assists patients in identifying and applying for appropriate cancer-related financial assistance grants.
- Provides education on Social Work role and services to medical providers, nurses, medical assistants, and other oncology staff members.
- Works with the Social Work team to improve Oncology Service Line and Cancer Support Services programs through involvement in process improvement projects. Designs and disseminates cancer-focused psychoeducational materials for patients.
- Implements and Monitors the Distress Screening process.
Requirements
Minimum Required Education: Master's degree in Social Work.
Required Licensure: Colorado State licensure as a Licensed Clinical Social Worker (LCSW).
Minimum Experience: Outpatient Clinical Social Work: 1-2 years
Preferred Experience: Clinical Social Work in Oncology: 1-2 years

Medical Social Worker
