The Recreation Therapist will also be responsible for liaising with family members and participating in interdisciplinary meetings and conferences.
Duties and Responsibilities 1. Responsible for the individual assessment of residents using a standardized Therapeutic Recreation tool. 2. To develop individualized intervention plans based on the assessment results and in collaboration with the residents, family and the interdisciplinary team. 3. Responsible for documenting assessments, intervention plans, outcomes, progress, attendance and evaluation on the resident’s chart. 4. Oversees that attendance records contain descriptive measures of resident’s participation. 5. Recreation Therapist will ensure that programming for individuals meets standards of practice and includes routines of daily living as well as specialized activities. 6.
