The Clinical Specialty Coordinator works across the continuum to develop, implement, and support quality improvement efforts that improve outcomes for the specialty practice. Areas of focus include program development, standardization of practice and documentation, communication, sharing of information, hand-offs, transitions, advocacy, patient and family guidance, education, resources, and specialty specific expertise. The Clinical Specialty Coordinator provides guidance and support to staff in the management, provision and coordination of care in their area of expertise.
The nurse care coordinator will support the TRUE (Trust, Respect, Understanding, Emerge) Center. The TRUE center is a fast growing clinic that serves children and adolescents who identify as transgender or gender diverse. The nurse will work closely with a multi-disciplinary team which includes physicians, nurses, dietitians, social workers, psychologists, medical assistants and outside agencies. The nurse care coordinator will also work at times with the after-hours/triage nurses, One Call operators, and schedulers. Good communication and organizational skills are essential in this role.
