Team-based care will be delivered from Patient Medical Homes in Patient Medical Neighborhoods, a concept developed and endorsed by the College of Family Physicians of Canada (CFPC). A Patient Medical Home is “…a family practice defined by its patients as the place they feel most comfortable - most at home - to present and discuss their personal and family health and medical concerns...”. A Patient Medical Neighborhood is a network of care involving all health care providers and services outside of the Patient Medical Home. Patient Medical Homes coordinate and integrate access to services within the Neighborhood and community for care of the patient.
