Assess cases presented to you by your Care team and utilizing your clinical competencies, prioritize member needs
Interface with specialists, hospitals, and community based organizations to facilitate collaboration in service of our members, and promote shared decision making
Monitor and address clinical quality gaps in care
Utilize our custom-built care facilitation platform, Commons, and the market’s EMR to collect data, document member interactions in the field, organize information, track tasks, and communicate with your team, members, and community resources
Attend daily team huddles and weekly case conferences to advise on physical health needs and establish health goals as part of member’s Member Action Plan (MAP)

Hospital schedule: 2 hospitalists daytime, 1 nocturnists (1-3 shifts per month average)
Manageable workload allows focus on consistently exceptional care, communication with families and with specialists who are outside of the area. Average inpatient load 10-15. Combination of med/surg, consults, and lower acuity ICU. Average outpatient load 12-14 visits.
Must be comfortable with/be excited about critical care/ventilator management, managing patients without specialists readily available though remote specialities are available (Intensivist, Pulmonary Medicine and Nephrology).
Competitive salary, collegial subspecialty availability (surgery, GI, orthopedics, neurology, cardiology), supportive administration.
Inpatient shift pay (12 hours): 60% for California
Outpatient shift pay (8 hours): 60% for California
Board certification in Internal Medicine, Family Medicine or Hospital Medicine and ACLS required.
