SUMMARY: This position involves 0.5 FTE of Prevention Nurse responsibilities and 0.5 FTE as a Care Team RN (5 half days per week in each role).

As a Prevention Nurse, this individual helps implement preventive health activities at Esperanza Health Center. The Prevention Nurse provides education, support, and clinical follow-up for patients who are prescribed PrEP (pre-exposure prophylaxis for HIV). The Prevention Nurse works with the Clinical Medical Director to support chronic care initiatives, including diabetes care management and education, mental health screening initiatives, and other targeted screening activities as directed. Together with primary care practitioners and outreach coordinators, the Prevention Nurse will develop and implement care plans and patient education, and work with team members to provide linkages for the various health and social needs of patients.
