Crisis Therapist - Emergency Mobile Psychiatric Services
Community Health Resources ABOUT THE PROGRAM: Emergency Mobile Psychiatric Service (EMPS) EMPS provides crisis telephone triage & support, mobile crisis assessment and intervention in the school, h... ome, court and other community settings. EMPS supports children & families in times of emotional, behavioral, and situational stress. EMPS focuses on prevention and early intervention, stabilization and diversion from hospitalization.
DUTIES & RESPONSIBILITIES: - Provides crisis assessment and follow up to monitor status of clients in crisis. - Documents ongoing client progress and outcomes - Demonstrates competency in age related or specialty issues and developmental needs for each population served. - Works with clients to develop appropriate discharge plans utilizing natural supports. - Maintains collaborative relationships with family members and other service providers. - Performs home and community-based care. - Responsible for maintaining client and staff records according to agency policy. - Participates in weekly supervision and meetings to review client and program progress. - Additional duties as required.

Tasks include telephone triage, mobile crisis assessments, risk assessments, de-escalation, safety planning, working with EMS as needed, facilitating referrals to emergency department, community mental/substance abuse treatment and other linkages in the community.
