Federally Qualified Health Center in beautiful Northern California. Shasta Community Health Center is a Healthcare for the Homeless funded program. This 4-5 day per week position offers a competitive salary and benefits package, providing medical care, addiction treatment and support to individuals in our community experiencing homelessness. At Shasta Community Health Center, the HOPE Program (Health Outreach to People Everywhere) platforms of care include: Mobile Outreach Van, Street and Encampment Outreach, Medical Respite, Shelter-based clinic site and hospital transition-of-care coordination.

This position works closely with the Medical Director of HOPE, a board certified Family Physician with fellowship experience in Outreach Medicine, as well as several other Health Center providers. Support staff include Medical Assistant, case manager, Scribe, Drug and Alcohol Counselor and Behavioral Health Consultant. The provider will also fill occasional Urgent Care shifts in the main Health Center site.

​​​This position offers opportunity for teaching medical students, medical residents and physician assistant/ nurse practitioner fellows participating in post graduate clinical training. Our training programs are UC Davis affiliated.

The Registered Nurse will have primary responsibility for improving the overall health of our street medicine program participants including promotion of physical and mental health integration. The Registered Nurse will work with members of the multi-disciplinary Street Medicine Team and our supportive housing and outreach teams towards this goal by engaging in the following activities:

• Conducting diagnostic and risk assessments and providing care in multiple settings including in institutional settings, participants’ homes, our medical suites, and on the streets
• Providing clinical care, including administering medication and performing wound care, at the direction of the Street Medicine and onsite Primary Care Providers
• Working collaboratively with patients to identify and overcome barriers to treatment
• Linking participants to primary and specialty care
• Participating in multidisciplinary team meetings
• Liaising with pharmacies, hospitals and city agencies as needed

Janian’s primary care program, started in 2012, continues to expand rapidly. Janian providers work in a variety of settings including supportive housing, shelters, safe havens, drop-in centers, and with street outreach teams. In July 2016, Janian launched The Street Medicine Team in Manhattan to provide primary care services to people experiencing street homelessness who are unable to access healthcare in more traditional settings. The initiative was quickly expanded to a second team serving similar clients in Brooklyn and Queens. In July of 2020, DHS announced it would again be expanding the initiative to address further need for such services. The Street Medicine Team works with people for as long as is necessary with a focus on quality of care provided over quantity of clients served. Each team consists of 1 NP/MD, 1 RN, and 1 MA/van coordinator. The teams work closely with outreach teams who identify and refer medically vulnerable clients.

Remote Grant and Partnerships Coordinator Volunteer Position

The Refugee Health Alliance is looking for dedicated health justice advocates with excellent writing skills to become Grants and Partnerships Coordinators (GPC)! As thousands of migrants are lining up along the Tijuana border in the hopes of finding refuge from being violently persecuted in their hometowns under dire conditions, the need for donations to meet the needs of this population has mushroomed. A GPC assists with writing and applying for grants, fundraising for the work we do in Tijuana, and helps in networking and collaborating with organizations in the US and Tijuana. Previous grant writing experience is preferred, but not required. You can find a list of responsibilities and preferred skills below. The Refugee Health Alliance has been providing the majority of medical care and basic needs for asylum-seekers and other vulnerable populations at the US-Mexico border in Tijuana since 2018 (e.g., 700 migrants a month). Our work has been recognized by accolades and awards from organizations like Al Otro Lado. We have 2 weekday clinics in Tijuana, provide remote services, and visit migrant shelters on the weekends. We provide a variety of ongoing, trauma-informed services including medical, midwivery, mental health, dentistry, acupuncture, naturopathic medical, and gender affirmation and LGBTQ services. When we can meet a need ourselves, we provide acute care and medical legal assistance and referrals. We also work to address the social determinants of health by distributing food and donations, and building Tijuana’s first ever clean public drinking fountain.
