Community Health Worker- Prenatal

The Community Health Worker will manage a caseload of pregnant individuals and their families to provide support throughout the pregnancy and connection to community resources. Will serve as a liaison to navigate/link them to health and social services in the community and coordinate community-based care referrals to meet their needs. Will provide culturally appropriate support and information. Emphasizes the importance of a medical home and proper utilization of the emergency department and outpatient clinics. Builds individual and community capacity by increasing health knowledge and self-sufficiency through a range of activities such as outreach, community education, informal counseling, social support, and advocacy. May provide information on available resources, provide social support and informal counseling, and advocate for individuals and community health needs.
