Responsibilities include, but are not limited to the following:

Prepare court documents in a timely manner, attend court hearings and testify as requested.
Develop service plans, make referrals and work with clients to complete tasks.
Document client contact in SACWIS DCFS software system in a timely manner.
Complete in-home visits and community visits to monitor progress of biological parents and children in care per the requirements of DCFS programs.
Monitor well-being of clients using an integrated health approach. Make referrals as necessary.
Assess safety and risk of all children in substitute care.
Coordinate visitation between children and their biological parents; which may include transportation and supervision of visits.
Coordinate and arrange services with community providers as appropriate.

Participate in on-call rotation. Respond appropriately in crisis situations.

Responsibilities include, but are not limited to the following:
Contribute to the design, implementation, and evaluation of Medication Management System including medication administration by non-licensed staff, proper storage, reconciliation and accountability of medication, assure necessary medications are available to individuals
assure best practices and policies are being followed in implementation of medication administration process, assure necessary medication diagnostic monitoring occurs as recommended or prescribed education to clients and staff regarding medication, treatments, side effects to monitor for conduct related quality assurance activities, applying feedback from medication error data. Instruct and evaluate on proper medication administration
Responsible for implementation and oversight of all health and psychiatric services to children and youth served by Chaddock.
Act as health services liaison between Chaddock and health care providers both locally and nationally as directed by supervisor.
Act as health services liaison and oversee health services coordination with the parents/guardians of clients served at Chaddock.
Responsible for implementation and oversight of medication management policies, procedures and practices for Chaddock.
Implement and oversee health services for both clients and staff relative to licensing, accreditation and Chaddock policy and procedure, ensuring agency compliance with those standards.
Participate in nurse on call rotation.
Complete documentation of health services that is compliant with the Nurse Practice Act, licensing, accreditation and Chaddock policy and procedures.
Demonstrate a working knowledge of trauma, attachment and child development needs for children and youth.
Ensure accountability to agency and external time lines and quality of health services.
