Responsibilities/Essential Job Functions
o Confirm Program eligibility.
o Verify the required documents for the application.
o Conduct pre- and post-household assessments.
o Submit payment requests for rental and utility payments on the client’s behalf.
o Collaborate and coordinate the integration of services with partners with the goal of assisting households in obtaining economic stability.
o Maintain and verify records documenting individual participant activities, progress, and other services necessary to achieve case management goals, as needed.
o Enter client information into required data entry systems.
o Ensure all required documents are complete.
o Attend staff and community meetings aimed at building relationships and mobilizing resources.
