Day Center Supervisor LPN

LPN license
Bachelors Degree or equivalent education and experience.
Three years experience in a geriatric health setting including a minimum of two years of demonstrated successful supervisory experience. Management experience preferred.
Proficiency with various business software, preferably Microsoft Office.
Must have a valid driver's license, proof of insurance and have means of transportation.
Must have medical clearance for communicable diseases and up-to-date immunization before having direct participant contact.
Skills and Knowledge:
Knowledge of principles, practices standards and techniques of a Certified Nursing Assistant (C.N.A.).
Working knowledge of the PACE regulations and Colorado regulations.
Working knowledge of healthcare and aging networks.
Working knowledge of physical, mental, spiritual, and social needs of the frail elderly and their families.
Working knowledge of quality improvement and cost containment systems.
Demonstrated effective leadership and decision making skills.
Demonstrated strong facilitation and conflict resolution skills.
Ability to foster and build professional relationships among staff.
Ability to effectively and efficiently plan, prioritize and follow-up on/delegate responsibilities.
Effective written and oral communication skills.
Working knowledge of local health care and geriatric service networks.

Works with various vendors to meet medical equipment needs of participants. • Maintains professional code of ethics and state licensure laws. • Supervision of Physical Therapist Assistant in accordance to state licensure laws. • Keep work area and equipment clean and orderly by maintaining and practicing hospital and department policies and procedures regarding safety and infection control. • Performs as member of PACE team through joint interventions, communication, quality assurance programs and meetings.
