LIFE Pittsburgh LIFE Pittsburgh is a vibrant PACE program in southwestern Pennsylvania. We currently serve close to 600 participants at 4 sites. We are looking to add a seventh PCP to our six member primary care team. LIFE Pittsburgh’s primary care team is composed of 3 physicians and 3 CRNP’s. We are looking for another provider who loves this model of care - physician or APP.
Day Center Supervisor LPN
Senior CommUnity Care - VOA Under the supervision of the Center Manager, manages the daily operations of the Senior CommUnity Care Adult Day Health Center in conformance with Center policies, procedures, and standards and all governing state and federal regulations. Responsibilities include the planning, coordination and implementation of all activities in the Day Health Center as well as the supervision of the Day Center Workers.
Bachelors Degree or equivalent education and experience.
Three years experience in a geriatric health setting including a minimum of two years of demonstrated successful supervisory experience. Management experience preferred.
Proficiency with various business software, preferably Microsoft Office.
Must have a valid driver's license, proof of insurance and have means of transportation.
Must have medical clearance for communicable diseases and up-to-date immunization before having direct participant contact.
Skills and Knowledge:
Knowledge of principles, practices standards and techniques of a Certified Nursing Assistant (C.N.A.).
Working knowledge of the PACE regulations and Colorado regulations.
Working knowledge of healthcare and aging networks.
Working knowledge of physical, mental, spiritual, and social needs of the frail elderly and their families.
Working knowledge of quality improvement and cost containment systems.
Demonstrated effective leadership and decision making skills.
Demonstrated strong facilitation and conflict resolution skills.
Ability to foster and build professional relationships among staff.

Community LIFE Community LIFE, a Program of All Inclusive Care for the Elderly (PACE®) in Pennsylvania, is seeking a Medical Director to join the Executive Leadership Team. The Medical Director is responsible for: recruiting, training, and leading the primary care staff at Community LIFE health centers; advancing the integrated care model with palliative and behavioral health; and educating our network partners in care and the community on the program and its benefits. The Medical Director is responsible for the medical care delivered, and oversight of the quality program and health outcomes. The LIFE model of care is an interdisciplinary care model which provides long-term living supports, routine and acute health care in a fully integrated manner.
PACE of Southwest Michigan Responsible for all medical director administrative duties, train and mentor PCP staff, deliver geriatric primary care.
McGregor PACE Fully integrated PACE program with 3 geriatricians and 6 APNs. Manage 4 PACE teams (approximately 220 participants) with 2 full-time NPs .
Northland PACE Physician to serve as primary care provider in a new PACE site in Fargo ND. We currently have 2 other physicians and 1 NP which cover other ND sites (Bismarck, Dickinson, and Minot). Looking for a collaborative team player to help lead our clinical team!
PACE Primary Care Physician

Community LIFE The Education Coordinator is responsible for working with the Education Supervisor to administer, organize and conduct company training programs for staff to meet program needs and regulatory requirements. Additionally, the Education Coordinator will be responsible for staff wellness education.
Required Education: Graduate of an accredited nursing school.
Community LIFE Under the direction of the Director of Quality Improvement, the Quality Coach collects, monitors and reports outcomes in collaboration with multiple disciplines. To assure highest quality of care and adherence to required documentation as define by regulations and organizational polices, this person will audit health records and other clinical documentations and present findings in an understandable format to appropriate disciplines. He or she will be a resource and provide consultation to other disciplines. Additionally, the Quality Coach will identify areas of concerns and assists in developing a corrective action plan.
Required Education: Pennsylvania licensed Registered Nurse; knowledge of performance/quality improvement concepts. BSN or MSN Preferred.
Chief Medical Director of Florida PACE Centers
Miami Jewish Health/ Florida PACE Centers Lead and coordinate Medical Services in 4 PACE sites and 20 providers within a 900 participant program. Siouxland PACE The PACE Physical Therapist functions as an integral part of the PACE team and has a responsibility for evaluating and treating patients requiring services associated with physical therapy. The Physical Therapist is expected to communicate with the PACE Interdisciplinary Team regarding the patient’s treatment plan and progress, and deliver services consistent with policies and procedures governing the delivery of patient care by the PACE program Essential Functions/Responsibilities: • Perform examination and evaluation to determine diagnosis and prognosis of patient/client and determine appropriate intervention to gain expected outcomes based on patient/client goals. • Utilization of appropriate Physical Therapy Interventions to produce expected outcomes. • Utilization of appropriate documentation to communicate progress and status to PACE interdisciplinary team members, including physician, nurses, social workers, occupational therapist, recreation, dietary, home care, etc., in order to coordinate and manage care of PACE participants living at home, a nursing/skilled nursing facility. Follows through with recommendations of IDT team. Works with various vendors to meet medical equipment needs of participants. • Maintains professional code of ethics and state licensure laws. • Supervision of Physical Therapist Assistant in accordance to state licensure laws. • Keep work area and equipment clean and orderly by maintaining and practicing hospital and department policies and procedures regarding safety and infection control. • Performs as member of PACE team through joint interventions, communication, quality assurance programs and meetings.
