Day Center Supervisor LPN
Senior CommUnity Care - VOA
Under the supervision of the Center Manager, manages the daily operations of the Senior CommUnity Care Adult Day Health Center in conformance with Center policies, procedures, and standards and all governing state and federal regulations. Community LIFE, a Program of All Inclusive Care for the Elderly (PACE®) in Pennsylvania, is seeking a Medical Director to join the Executive Leadership Team. The Medical Director is responsible for: recruiting, training, and leading the primary care staff at Community LIFE health centers; advancing the integrated care model with palliative and behavioral health; and educating our network partners in care and the community on the program and its benefits. The Medical Director is responsible for the medical care delivered, and oversight of the quality program and health outcomes. The LIFE model of care is an interdisciplinary care model which provides long-term living supports, routine and acute health care in a fully integrated manner.

Quality Coach
Community LIFE
Under the direction of the Director of Quality Improvement, the Quality Coach collects, monitors and reports outcomes in collaboration with multiple disciplines. To assure highest quality of care and adherence to required documentation as define by regulations and organizational polices, this person will audit health records and other clinical documentations and present findings in an understandable format to appropriate disciplines. He or she will be a resource and provide consultation to other disciplines. Additionally, the Quality Coach will identify areas of concerns and assists in developing a corrective action plan.

Required Education: Pennsylvania licensed Registered Nurse; knowledge of performance/quality improvement concepts. BSN or MSN Preferred.

Physical Therapist
Siouxland PACE
The PACE Physical Therapist functions as an integral part of the PACE team and has a responsibility for evaluating and treating patients requiring services associated with physical therapy. The Physical Therapist is expected to communicate with the PACE Interdisciplinary Team regarding the patient’s treatment plan and progress, and deliver services consistent with policies and procedures governing the delivery of patient care by the PACE program Essential Functions/Responsibilities:
• Perform examination and evaluation to determine diagnosis and prognosis of patient/client and determine appropriate intervention to gain expected outcomes based on patient/client goals.
• Utilization of appropriate Physical Therapy Interventions to produce expected outcomes.
• Utilization of appropriate documentation to communicate progress and status to PACE interdisciplinary team members, including physician, nurses, social workers, occupational therapist, recreation, dietary, home care, etc., in order to coordinate and manage care of PACE participants living at home, a nursing/skilled nursing facility. Follows through with recommendations of IDT team. Works with various vendors to meet medical equipment needs of participants.
• Maintains professional code of ethics and state licensure laws. • Supervision of Physical Therapist Assistant in accordance to state licensure laws.
• Keep work area and equipment clean and orderly by maintaining and practicing hospital and department policies and procedures regarding safety and infection control. • Performs as member of PACE team through joint interventions, communication, quality assurance programs and meetings.
