Facility Cook
Hawai'i Care Choices
The Facility Cook collaborates with the Administrative Team Lead to provide a variety of meals for patients, staff, and in-house functions. The cook must have an understanding of diverse food cultures and be knowledgeable in food storage and preparation according to health and safety standards and dietary requirements of the seriously ill.

Duties & Responsibilities.

Prepares food efficiently while ensuring that the presentation of each food item is taken with the utmost care.
Ensures that state and federal regulations are followed to maintain a clean and safe environment for the storage, preparation, and handling of food.
Demonstrates flexibility to accommodate patient preferences and dietary requirements as dictated by each patient’s medical condition.
Coordinates related expenditures in a fiscally responsible manner in accordance with the kitchen’s budget.
Communicates and facilitates all kitchen-related office administration activities and third-party contractors.
Maintains accurate food-ordering and stocking levels, including all kitchen materials.
Organizes, coordinates, and delegates responsibility to care center volunteers to ensure goals and objectives of the kitchen are met.

Skill & Knowledge Requirements​

Knowledge of safe food preparation and handling procedures, sanitation standards, production processes, and proper food storage.
Ability to use all kitchen equipment safely and properly.
Compliance with accepted professional standards and practices.
Basic working knowledge and skill with computers, such as email.

Educational and Experience Requirements

Minimum high school diploma or equivalent.
Minimum 2 years cooking experience in a professional setting.
Current food handler’s card from DOH-State of Hawaii.

Hours

Registered Nurse - Float/Per Diem/Case Manager

Position Summary:
Responsible for the coordination of all hospice care. Provides skilled nursing care in accordance with the treatment and plan of care prescribed by patient, family, physician, and members of the interdisciplinary team. The Float Registered Nurse, will provide staffing support to cover admissions, scheduled visits, referral/intakes, crisis care, and/or assist with patient care in other facilities/venues as needed.

Duties & Responsibilities​
Helps to ensure timely admission of referrals to various HiCare programs (hospice, inpatient and contracted facilities).
Provides backup for HiCare’s various service lines (hospice homecare, inpatient, palliative care).
Empowers and supports care givers and patients through education of the disease process and prescribed care regimens
Ensures the safe custody and administration of drugs in accordance with the Standards of Practice
Assists in maintaining the 24 hour on-call coverage as needed.

Skill & Knowledge Requirements​
Excellent observation, critical thinking, assessment, nursing judgment and communication skills
Comfortable in maintaining professional boundaries
Interest and desire to give hospice care with a special sensitivity to the needs of the dying.
Sound working knowledge of hospice Conditions of Participation (COPs) and clinical standards of practice

Educational and Experience Requirements
Graduate of a registered nursing curriculum; BSN or higher preferred
Two (2) years experience in a healthcare environment or equivalent position
Possession and maintenance of a valid Hawai'i State nursing license
Valid CPR certification

Hours

Regular full-time employee. Block shifts (may start mid-day at times). Schedule determined by Supervisor.
Duties may require after hour work including weekends and holidays, as needed.

Certified Nurse Aid - Homecare

Position Summary

Follows assigned duties and instructions as delegated by the Hospice RN and Clinical Team Leader. Is responsible for providing a variety of direct patient care support services and tasks.
Completes patient visits as scheduled and assigned.
Documents consistently all care, services and observations in accordance with hospice protocol and keeps RN informed of changes in patient’s condition.
Employs appropriate and safe techniques in performing personal hygiene, grooming tasks, assistance in activities of daily living and CNA services to hospice patients in compliance with federal, state and hospice regulations.

Skill & Knowledge Requirements​

Comfortable in maintaining professional boundaries.
Ability to work with patients and families coping with a terminal illness.
Ability to independently provide direct personal care services to patients in their homes or in a facility setting.
Must have good communication skills both verbally and non-verbally. Ability to deal with people in crisis in a mature and non-judgmental manner.

Educational and Experience Requirements

State Certification from a State of Hawai’i Certified Nurse Aide Training Program.
At least two (2) years CNA experience in a facility healthcare setting or equivalent preferred.
Valid CPR certification.

Hours

Primarily Monday to Friday, 8:00 a.m. to 4:30 p.m., with possibility of occasional evening, weekend and holiday work.

Spiritual Counselor

Position Summary

Provides spiritual counseling and support for Hawaii Care Choices patients and families, in keeping with the patient’s and families’ personal desires and belief system(s).
Acts a liaison and educator between Hawaii Care Choices and community clergy and their organized faith community.
Directs and provides spiritual care for the Interdisciplinary Team.
Participates in memorial/funeral services as requested for those families not affiliated with a church or those who request participation.

Skill & Knowledge Requirements​

Highly skilled and self motivated individual with the ability to exercise independent sound judgment in decision making, planning and provision of quality spiritual care and education.
Possession of critical assessment and thinking skills.
Ability to embrace people from different ethnicities, beliefs and faith traditions.

Educational and Experience Requirements

Bachelors’ degree or equivalent with experience in/and a desire to provide spiritual care.
Adequate educational and experiential levels to meet the needs of the patients, care givers and regulations established by state and federal laws.
Experience providing bereavement and spiritual care in a hospice type setting preferred.
