Outreach Specialist
SEARCH Homeless Services
Face to face outreach, assessment, engagement and navigation activities primarily delivered in the field throughout Houston and Harris County. Daily trips to meet with clients and prospective clients in encampments, under bridges, in alleys, and visiting other locations not fit for human habitation. Based out of SEARCH’s HQ building, spends frequent time in house as well for supervision, documentation, training, and collaboration on shared client cases. Utilizes Evidence-Based Practices, such as the Transtheoretical Model and Motivational Interviewing, as appropriate to connect with and motivate these high need, high vulnerability clients to connect with our intensive case management services via PATH and/or connections to navigation, disability, or other programs as appropriate. Facilitates client de-escalation as needed. Works with and motivates individuals who present with physical health, mental health, and/or substance use disorders.
Bachelor’s Degree preferred with 2+ years prior work experience with dual diagnoses clients. Texas Driver’s license with acceptable 3-year safety record required.

DMD Clinical Case Manager
SEARCH Homeless Services
Provides street outreach and case management services to individuals experiencing literal homelessness within the Downtown Management District’s preferred geographic zone in order to fulfill SEARCH’s mission of providing hope, creating opportunity, and transforming lives. Provides engagement, assessment, and referrals to outreach clients while providing intensive case management to a targeted caseload. Provides ongoing case management and service planning to clients who’s experience of homelessness in Downtown Houston has been exacerbated by complex barriers. Assists with service planning and coordination within The Way Home system in order to transition clients into permanent living situations. Assists clients with reducing barriers to housing, medical and mental health treatment, substance use treatment, and income. Provides hands-on assistance with facilitating clinic appointments, ID replacement, criminal background mitigation, and more. Transports clients to prospective housing sites, clinic appointments, and other referrals as necessary. Utilizes Evidence-Based Practices, such as the Transtheoretical Model, SOAR, and Motivational Interviewing, as appropriate to assist these high need, high vulnerability clients in making and sustaining change. Works with individuals who present with physical health, mental health, and/or substance use disorders to build motivation and achieve individualized goals. Other related duties as assigned.
Master’s Degree in Social Work or related Counseling field, with a state licensure of LCSW or LPC required. 1-3 years in social service setting desired; prior work or internship experience with homelessness, mental illness, substance use disorders, and dual diagnoses strongly preferred. Excellent interpersonal and communication skills. Cultural competence and Motivational Interviewing preferred.

- Coordinates and implements quality management (QM) and continuous quality improvement (CQI) activities and leads planning efforts to improve and enhance delivery of quality HIV services.
- Oversees a structured, system-wide approach for quality management and service delivery planning.
- Performs complex analysis of performance data including health outcome, compliance, patient satisfaction, and epidemiologic data.
- Prepares reports, develops requests for proposal/quote and assists with federal and state grant applications.
- Provides quality assurance oversight specific to HIV medical and support services provided by subrecipients.
- Responsible for developing, implementing, monitoring, and updating the Quality Management Performance Work Plan.
- Provides technical assistance and guidance to subrecipients to ensure compliance with applicable federal, state, and local regulations.
