Clinics Nurse Supervisor

St. Luke Community Healthcare is seeking a high-performing, full-time Clinics Nurse Supervisor responsible for supervising, mentoring, and overseeing the daily nursing operations for our primary care providers at four separate locations. The candidate must possess the ability to constructively problem solve utilizing sound critical thinking skills and effective communication. Primary responsibility includes leadership of the nursing staff to ensure quality, safety, and consistency in delivering healthcare that meets and exceeds patient expectations, nurse team staffing, supervising, evaluating, and providing performance feedback to nurses, identifying and resolving operational problems, and partnering with physicians and providers at the department level to achieve clinical, operational, and service goals.

The candidate must possess a current State of Montana RN license; 2 – 3 years experience working in medical practice; computer proficiency. Supervisory experience is desirable. Experience working with an ambulatory electronic medical record is expected.

Licensed Clinical Social Worker

The ideal candidate would possess a Masters of Social Work degree, a current Montana LCSW license and two years of social work experience in a healthcare setting or clinical experience in a psychiatric setting or behavioral health practice, and familiarity with substance abuse problems.

Ideally, you will assist primary care providers in managing the overall health of their patients. The LCSW will be responsible for the management of psychosocial aspects of chronic and acute diseases, apply behavioral principles to address lifestyle and health risk issues with an emphasis on prevention and self-help approaches, partner with patients and primary care providers in treatment approaches, and assist with developing a fully integrated, multidisciplinary model within our primary care clinics.

The BHP specializes in brief, evidence-based, solution-focused assessments and interventions geared toward functional restoration and empowerment. The BHP typically sees patients for 15 to 30 minute appointments and care may address chronic health conditions (e.g. diabetes, obesity, and chronic pain), mental health and substance use conditions, health behaviors, life stressors and crises, stress-related physical symptoms and ineffective patterns of healthcare utilization. Behavioral health services include assessment, treatment planning/coordination, collaboration, monitoring, review and advocacy. Adapts interventions to the work flow and pace to that of primary care, including rapid assessment, brief treatment, and a high daily volume of contact.
