The team is focused on working with partner hospitals and referring agencies to provide for the acute and ongoing medical respite for patients, providing short-term supportive housing, crisis management, referral to appropriate substance abuse treatment, physical health support, and mental health treatment referral, access to benefits and entitlements, appropriate support to secure safe, affordable, permanent housing, as part of a comprehensive approach to ending homelessness and achieving self-sufficiency. Coc Youth Coordinated Entry Administrator

The CoC Youth Coordinated Entry Administrator is responsible for expanding and supporting the Memphis/Shelby County youth coordinated entry system and team of Youth System Navigators. This includes providing year-round support to the youth serving agencies and collaborating on all CoC related committees and activities.

Continuum of Care Planning Administrator

The Continuum of Care (CoC) Planning Administrator will report to and partner closely with the CoC Planning Director to administer all aspects of CoC planning activities. This includes assisting the CoC Planning Director with year-round planning support to the Memphis/Shelby County Homeless Consortium, technical assistance, and oversight of all CoC related committees; and assistance with community Strategic Plan to End Homelessness activities.

Continuum of Care Housing Navigator

The Housing Navigator will assist individuals and families in moving into permanent housing by working closely with prospective landlords, partner landlords, program participants, Memphis Housing Authority, and partner agencies (case managers and program directors).

CoC Youth Housing Navigator
Community Alliance for the Homeless
The Youth Housing Navigator will assist homeless youth (18 – 24 years old) in moving into permanent housing by working closely with partner agencies (Case Managers and Program Directors), Memphis Housing Authority (MHA), prospective landlords, partner landlords, and program participants. The Youth Housing Navigator will also work closely with the Youth System Navigators team to ensure flexible funds are distributed and partnerships are built to remove barriers for youth when accessing stable housing.
