Complete Department of Health Services (DHS) functional screen to determine functional eligibility. Assist consumer with obtaining physician’s prescription as necessary. Ensure that all required documentation is complete and filed in a timely manner. Assist consumers with selecting a service facilitation agency and facilitate a warm transfer to that agency for ongoing services. Assess need for immediate services and make arrangements if necessary. For those determined ineligible or not interested in CCS, provide information and assistance in accessing alternative resources. Perform annual reassessments of eligibility with DHS functional screen. Provide community education on the CCS program. Outreach to other service access points. Perform other duties as assigned.

About the PRN position: Assigned shifts “as
needed” and does not have a regularly set schedule, but must work a minimum of one (1)
8-hour shift once per quarter to maintain PRN employment status with SOAR. When the

the next assigned shift.
