Patient Care Navigator
The Road Home Program The Patient Care Navigator (PCN) is the first point of contact for all clients seeking mental health care at RHP and thus a vital role and member of the RHP team. PCNs register pa... tients for services, assist them with navigating the electronic health record system, help obtain medical records, and schedule initial appointments. They assist clinicians with case management duties and are a critical part of the team that manages logistics prior to an individual attending the IOP. Individuals in this role will manage a large amount of patient data and thus must be organized and able to track a caseload of patients. The PCNs maintain regular contact with all individuals accepted to the IOP, conduct suicide and substance use coping plans, and conduct outreach calls once clients complete programming. RHP serves a broad array of active duty military personnel and veterans with minoritized ethnic or racial identities, LGBTQ+ identity, low socioeconomic status, and other minoritized identities. #2. Participating in and planning project management for various Information Technology (IT) modernization projects; Preparing requirements for IT systems- A bit more on this, tasking at MEPCOM shifts very quickly and the ideal hire would be able to understand how to dedicate resources to best suit the situation and manage unplanned setbacks to still accomplish the mission.
#3. Training and tasking the MEPS IT Service rep. This is complicated, but they (the ITS) do not work for us, yet, they are our greatest asset. So, identifying those ITSs that need help and standing up the mentor program with a J6 HQ technician in the customer service branch and helping the MEPS commanders understand WHY it is so important.
#4. Foresight and planning. This goes along with Matt Lince’s Hungry, Humble, People Smart, and Productive desires for J6 hires. For me, this is that feeling I get that there is a true “heart” desire to commit to the mission and that will be visible through how proactively they think. I feel that that is demonstrated through having lots of thoughts on how to handle any situation so that we are always ahead of issues, when possible.
